2 Dual Op-Amp to Quad Op-Amp Adapter

Looking for an adapter to use 2 dual op-amps in place of a quad op-amp?

I found this DIYSB where someone shows what pins need to be connected together, but I remember seeing one made by John Kallas and it looked much nicer and neater then this.
