Negative ground fuzz problem

The output on the pedal is crazy loud and the volume knob starts out above unity. Just wondering if i missed something.  Also i would like to add a tone knob to it and im not sure how to go about doing that.  Thanks!

Volume 1 should be connected to ground.

Thanks!  It's working great now.  Any ideas on how to add the tone control?
