WTH? OpAmp distorting signal...SOLVED

So I am building another Paisley drive, and using a JRC4580D OpAmp chip.  So the signal at pins 2 and 3 the signal is crystal clear...but at Pin #1 (and also at 5, 6, and 7) the signal is coming through stronger and much distorted.  At first I thought it was the chip itself, so I replaced it with an extra I had.  However, the new one did the same thing.

Any ideas what could be causing this?

Any help is appreciated.  My other Paisley drive came out amazing, so I am scratching my head.

Signal on grounded pin 4?
Miss the small link connecting the diodes to pin 1?
The 2K2 from pin 2 to Vref not 2K2?

A few high res pics and some voltages would better help

Apologies...there is no signal at Pin 4 (ground).  Doh!  Corrected original post.

BINGO...you nailed it with the 2.2K resistor...it wasn't!  And I looked all over the board and didn't see another 2.2k resistor anywhere, so they must have packaged the wrong resistor mistakenly (everything is in neat little bags).  But maybe not, it very well could have been my fault.

HOWEVER...after replacing with a 2.2K resistor, problem is still there.  As mentioned, the volume coming through Pin 1 is MUCH louder than pins 2 and 3.  But distorted.

The gain pot and the 2k2 are responsible for the amount of signal gain here.  So if everything else is correct I can only assume the gain pot or it's wiring are dodgy?  Confirm the pot itself is wired and functioning properly (measure between lug 2 and lug 1 as you turn the pot)

CH:  Wow.  Excellent stuff.  You are THE MAN!

I had visually inspected all of the wiring and solder points...but after your suggestion on focusing on the Gain pot, I physically manipulated all the connections again on that pot.  Turned out the Gain pot Lug #1 wire broke with the gentlest of movement.  So...a quick Clip, Strip, and Solder later the signal is now perfect!

Thanks again for the help!  Can't wait to drill the enclosure this is going into (the 1590B it's in now is just a wee bit too shallow to close up properly, so need a deeper box...going with a 125B like my previous one).
