I've searched and found some active DI box layouts to convert an unbalanced signal to a balanced XLR signal, but nothing for the other way around.

Do i just tap into pin 3 on the XLR input and run that into the 4-channel mixer? the other three channels are instrument inputs from guitar and bass.

Basically, yeah. You can in fact just get a XLR to PL cable and call it a day.

Would a ground lift be required for the XLR connector?
